CPE Status Report & Personal Particulars Update Form

The Singapore Pharmacy Council has mailed to all pharmacists with valid practising certificates (PC), a set of CPE report and Personal Particulars Update Form on 25 Sept 2009.

Pharmacists who did not receive this mailer are advised to notify SPC soonest possible.

Pharmacists are advised to update their CPE records via submission of claims online to ensure that they meet their CPE requirements before the end of the qualifying period (QP) on 31 August 2010. More information and resources on CPE can be found here.

All pharmacists should also update SPC on any changes in their personal and employment particulars, contact information and email address, either via their SPC account, or by submitting the Personal Particulars Update Form which can be downloaded here.

Under Pharmacists Registration Act 2007 section (15)(4), a registered pharmacist who failed to inform the Registrar of any change in his name, residential address, practice address, and other contact information within 28 days of the change shall be guilty of an offence and be liable to a fine not exceeding $1,000.
