Restoration to Register

Pharmacists who do not renew their PCs for a continuous period of 5 years or more will have their names removed from the Register of Pharmacists.

They must meet the following requirements to be eligible for restoration:

i) Already accumulated 25 CPE points (including a minimum of 8 Patient Care points) in the period of 12 months prior to the date of restoration.

ii) Sit for and pass the Forensic Examination [PDF, 0.144 MB] and Competency Examination.

iii) Submit the Application for Restoration Form [PDF, 0.175 MB] with Restoration Fee of S$300, and Practising Certificate Fee of S$500 for 2 years or part thereof, depending on the date of restoration.
